1. The website is www.Impacttestonline.com/BenHogan - 

1. The screen will have a Head (with Brain) with the words ImPACT Applications, Inc – you will have a place to enter the code below

2. Your code to enter is JMGG46676W

1. You will select Creekview HS

3. You will answer some demographic questions – below are answers that you must put in

1. Select ok for the first three pages until you get to Name and DOB

2. You may skip the address and language section

3. Years of Education completed will be 10

4. Current sport you plan to play at Creekview

1. Position does not matter

2. Leave level of participation – High School

3. Years of experience at this level will be – 2 years

5. If you have not had a concussion – you may skip those questions

1. If you did have a concussion - do not worry if your timeline or dates are exact

4. When you get to symptoms page – all symptoms should be zero unless you have a medical condition that causes a chronic symptom listed

5. You may enter the test

1. You will have 6 different tests + the last test 7th one that will repeat the words and shapes from the first test.  

2. Read the instructions carefully

3. If the program locks up - just re-start

4. When finished – a congratulations for completing the test will appear
